The Progress Notes Form is necessary to detail everything that happens during the course of treatment and between visits.
Write legibly using a blue or black ball point pen or ink. If abbreviations are used, be sure the code is handy so the abbreviation can be interpreted correctly. If a mistake is made, draw a single line through the error, write mistake above it, and then write the correct information following it.
