After years of discussion as well as careful thought and consideration, on May 1, 1985, a new patient recordkeeping system was introduced by the Wisconsin Dental Association at the WDA Annual Session. Because the charts were “developed by dentists, for dentists,” the recordkeeping system garnered immediate attention within the dental industry.
