| | To place an order you must:
1) Create an account and login to the website. (You can login through The Dental Record's Homepage.)
The login is used to create a unique account that only you can view with your username and password.
You will be asked to provide shipping and billing information as well as provide a method of payment.
*If you forget your password, please call 800-243-4675 and speak with a customer service representative to retrieve your password and account information.
2) You then choose a product category.
3) You should then select the particular product type and amount you would like to purchase.
4) At the bottom of the screen click on "update order".
5) You then add additional items the same way or view your order and check out by clicking on the "Current Order" tab at the top of each web page.
6) On the Current Order page you will have the option of updating or clearing you order, and checking out.
7) An e-mail confirmation will be sent to the address you supplied when the order is processed.

| | Orders placed before 12:00 noon M-F will usually be shipped the same day. All other orders will be shipped the next business day.
All orders are shipped via UPS Ground. In-stock orders take anywhere from 1-5 business days depending on where your business is located. (95% of items are considered "in-stock" items.) Special shipping arrangements are available if you need us to expedite the service. Please call 800-243-4675 to make special shipping arrangements.

| | When ordering online, you can pay using a major credit card (We accept Master Card, Visa, and American Express) or we can bill you. An invoice or statement will be sent to your billing address that includes shipping charges.

| | Customers who live in Wisconsin or Texas are required to pay their state sales tax with each Dental record purchase.
The Dental Record is a Wisconsin-based business. If your place of business or shipping / billing address is in Wisconsin you are required to pay Wisconsin state sales tax.
All purchases made by customers in Texas are required by Texas law to pay Texas state sales tax. (See statute 3.286 of the Texas tax code).

| | The Dental Record will ship to the United States, U.S. Territories, Canada, and Mexico. Special arrangements should be made for shipments outside of North America.

| | If your order is not correct, please call 800-243-4675. The customer service staff will have the correct shipment sent.

| | If your order has been damaged, save all your delivery receipts, packaging materials, and shipping containers. Please call us immediately at 800-243-4675 so we can file a claim with UPS. We will then see that a replacement order is shipped.

| | Our return policy is to refund the product price, less a 20% restock fee, within 90 days of the purchase date.

| | If items have been shipped incorrectly, damaged, or are defective, we will send a call tag to your office and the order will be shipped back to us. You are responsible for return shipping under other circumstances. If you need to return products they should be sent to our warehouse. You can obtain the warehouse shipping address by calling 800-243-4675. You must also re-pack and pad the products being returned, as the shipping boxes are not intended for multiple shipments.
