Informed Legal Consent Forms

Use the Consent To Dental Treatment Form with patients that need treatment beyond a regular check-up and cleaning.

Use the Consent Form for surgery or extensive services.

The Consent for Use of Restraint(s) form is helpful for pediatric and general dental offices when treating children, or when treating patients with a mental or physical handicap.

Use the Consent to Anesthesia form to document that you informed the patient about how the anesthesia will be administered, inherent risks, and possible complications.

The Informed Refusal form documents your patient's refusal of recommended treatment.
